First systematic NPPES address clustering and phone deduplication applied to Minnesota's $1B+/year Personal Care Assistance program. 4,817 PCA providers analyzed, 9 high-risk addresses (15+ enrolled providers), 12 shared-phone clusters flagged. Top 10 clusters ranked by provider count, enforcement history, and SOS anomalies. Referral letters drafted for top 3 clusters to HHS-OIG, DHS-SIRS, and all five MCO SIUs.
