After the July 2025 FBI raid on 1821 University Ave W (the Griggs-Midway Building), this investigation traces authorized officials of NEMT, HSS, and HCBS entities who relocated and enrolled successor entities at new addresses. Two confirmed successions documented: East African Healing Services → House of Opportunity LLC; Altitude Living Inc → Pine Branch LLC. DHS license cross-reference: Fadmetrocare Inc applied for HCBS license at SOS address (denied 10/09/2025); World Class Care LLC holds suspended HCBS license at Blaine address. Includes DHS-SIRS, HHS-OIG, and AG-MFCU referral letters for active MHCP enrollees.
