Twenty-three NPPES NPIs enumerated four months later, all assisted-living-facility taxonomy. Every backing LLC now inactive; every NPI remains federally active. Promoted from the Brand-Recycling Watchlist Tier-3 by the 2026-04-30 SOS scrape — the algorithmic Banadir criteria fired correctly, but the underlying single-batch single-agent pattern is more consistent with chain consolidation than unrelated-operator brand-recycling.
