Office of the Legislative Auditor program evaluation of Medical Assistance (Medicaid) program integrity in Minnesota. The earliest in the documented series of OLA audit reports warning DHS about systemic fraud vulnerabilities.
Significance: This 2007 report is the opening document in the 18-year chain of OLA warnings that DHS ignored — the subject of the 'OLA Ignored Warnings' investigation. It first recommended cross-program billing analysis and real-time claims monitoring. Neither recommendation had been fully implemented by the time the Feeding Our Future fraud occurred 15 years later.
